Happy Healthy People Living Active and Connected Lives. The Active Wellbeing Society (TAWS) is a community benefit society and cooperative working to develop healthy, happy communities living active and connected lives.

Iridium Medical Practices Social Prescribing Link Worker is Leila Yafai.

TAWS link workers offer on-going one to one support for individuals finding services they would like to access. They also co-design and deliver of a number of interventions and activities such as Knitter Natter, Talking Art, yoga, walking and cycling groups, proving community engagement opportunities.

Our TAWS link worker can support individuals who require help and support with

  • Increasing exercise or mobility
  • Social engagement
  • Weight management
  • Mild to moderate depression or anxiety
  • Frequently presenting at GP
  • Long term health condition
  • Healthy eating or nutrition
  • Challenging social circumstances Isolation or loneliness
  • Training/employment
  • Finding groups and activities
  • Sign posting to relevant organizations

Specialist Social Prescribing Service (Turning Point)

This service is provided by Turning Point.

Iridium Medical Practices Specialist Social Prescribing Link Worker is Aysha Amin.

Turning Point’s Specialist Link workers have been introduced to provide an additional level of support for individuals who may require extended support, outreach support, support to engage with services and attend appointments or have a range of complexities in their life that means they face chronic exclusion.

Our TP Specialist link worker can support individuals who require higher intensity support due to

  • Alcohol misuse
  • Misuse of drugs
  • Offending behavior
  • Learning Disability
  • Physical Disability
  • Anti-social behavior
  • Homelessness
  • Self-harm
